The Health Education Department's mission is to increase awareness of health concerns and to promote healthy life styles and behaviors in Cumberland County Residents.

This is accomplished though assessment of individual and community health needs.

NEW PROGRAM
    The Cumberland County Health Department
has a new chronic disease self-management program from Stanford University called “Take Control of Your Health.” This program is recognized worldwide as a cost effective evidence-based health promotion program for adults with chronic conditions and the caregivers of individuals living with a chronic condition. “Take Control of Your Health” allows a small group of adults of all ages to meet for 2˝ hours, once-a-week for 6 weeks to help one another cope with their health problems. The workshops are taught by trained certified Chronic Disease Self-Management trainers. Each participant will gain confidence and motivation to manage their health and maintain a fulfilling lifestyle by managing symptoms, communicating more effectively with their doctor, reducing frustration, fighting fatigue, and making tasks easier. Training is available to become a Certified Stanford University Chronic Disease Self-Management Peer Leader to facilitate “Take Control Of Your Health” workshops.  Individuals interested in becoming a Peer Leader must attend all 4 days of training. You do not need to be a health professional or have prior group facilitation experience to deliver the program. Peer leaders can be individuals who have chronic conditions or caregivers for people with chronic conditions.
