NEW

The Cumberland County Health Department
has
a new chronic
disease self-management program from Stanford University called “Take Control of
Your Health.” This
program
is recognized worldwide as a cost effective evidence-based health promotion
program for adults with chronic conditions and the caregivers of individuals
living with a chronic condition. “Take Control of Your Health” allows a small
group of adults of all ages to
meet for 2˝ hours, once-a-week for
6
weeks
to help one
another
cope with their health
problems. The
workshops are taught by trained certified Chronic Disease Self-Management
trainers. Each participant will gain confidence and motivation to manage their
health and maintain a fulfilling lifestyle by managing symptoms, communicating
more effectively with their doctor, reducing frustration, fighting fatigue, and
making tasks easier.
